 EMS in the Field

  • EMS Registration Form (Including Provisional/Temporary)
    • Fillable on-line
    • Submit via email with electronic signature
    • Emailed/faxed copies are acceptable
    • Temporary responders from out of state in good standing will need:
      • Proof of ID,
      • Proof of current licensure, and
      • A letter of good standing from the other state. 
    • There is not a fee for this license.
    • Licensure will be valid for the duration of the declared state of emergency. 
  • ALS Licensure Form (Including Provisional/Temporary)
    • Must obtain medical director signature
    • Emailed/faxed copies are acceptableFillable on-line
    • Temporary responders from out of state in good standing will need:
      • Proof of ID,
      • Proof of current licensure, and
      • A letter of good standing from the other state.
    • There is not a fee for this license.
    • Licensure will be valid for the duration of the declared state of emergency.
    • Advanced responders will need to have a medical director’s signature and an association with a response unit/squad or facility.
